Medicare and Chronic Care Management
CMS defines CCM as “care coordination services done outside of the regular office visit for patients with multiple (two or more) chronic conditions expected to last at least 12 months or until the death of the patient, and that place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline.”
Integrated Care Approach
As the basis of our Chronic Care Management Program, a concierge nurse is available 8:30am to 4:30pm Monday – Fridays. The program is patient centered where our goal is to help our patients maximize available resources and proactively manage health issues. Monthly phone calls with patient participation ensure better communication of health care goals and needs.

Advanced Planning – Office visits are arranged in advanced tailored to discussing chronic conditions, coping with disease management process, emotional safety and lifestyle.Direct Access to Registered Nurse – Jupiter Internal Medicine has assigned a Concierge Nurse coordinate patient planned visits, based on the patient’s needs, and self-management goals. Health Care Partnerships – Jupiter Internal Medicine Associates have partnered with other patient centered organizations to redesign care delivery resulting in better communication among disciplines and with other physicians and health care providers.
