Notice of Practice Transition and Changes to Insurance Participation

Dear Patient,

We are writing to inform you of a change to our practice regarding insurance participation.For the last 25 years, it has been our privilege to provide you with comprehensive, high-quality medical care. Many of you have trusted us with your healthcare for years, and some have even allowed us to care for generations of your family. Those relationships mean a great deal to us, which is why we want to communicate openly and honestly about an important change.

What is Changing? Effective, October 16, 2026, we are transitioning to a patient-focused model designed to offer enhanced access, longer appointment times, and highly personalized care. Depending on your current coverage and healthcare needs, you may choose from the following pathways to remain a patient with us: Traditional Medicare Patients

If you carry a traditional (Part B) Medicare, nothing changes. We will continue to participate in traditional Medicare, and we will bill your coverage as usual.

Concierge Internal Medicine

For patients seeking an elevated level of specialized care and 24/7 access, we offer our concierge internal medicine model led by Dr. Beatrice Deshommes and myself (Dr. Daniel L. Boss).

Concierge Women’s Health & Hormone Wellness Program

This program is led by board-certified gynecologist Dr. Susan Poncy. This option focuses on providing comprehensive, proactive health management for women who are beyond their childbearing years. Dr. Poncy specializes in general midlife and mature gynecology, preventative screenings, and personalized hormone replacement therapy (HRT) options tailored to support long-term wellness and vitality.

Direct Primary Care (DPC)

For non-Medicare patients with commercial insurance who wish to remain with the practice outside of a concierge membership, we are introducing a cash-based Direct Primary Care model. This subscription-based program provides comprehensive primary care services for a flat, predictable monthly fee without involving insurance companies.

If you wish to enroll in one of our new membership models, or if you have questions about how this transition impacts your specific coverage, please contact our office at (844) 537-6727 as soon as possible. To maintain the level of access and personalized care our patients expect, enrollment will be limited.If you choose to transition your care to a provider outside of our practice, we completely understand and will assist you in transferring your medical records to ensure continuity of care.

Why We Are Making This Change

In recent years, the escalating administrative demands imposed by commercial health insurance companies have increasingly restricted the time we can dedicate to what matters most, caring for you. Furthermore, the reimbursement rates we receive from these commercial insurance companies are no longer enough to cover our operational costs and keep the practice going. Unfortunately, this model is no longer sustainable for our practice. To preserve the high standard of personalized medical attention you deserve, seamlessly integrate your care with the hospital when needed, and ensure our practice remains viable, we are making a strategic shift in how we operate.We understand that change can be difficult, especially when it involves your healthcare.
